About this file
This is a certification form required by the City of Little Rock, Arkansas for vendors/contractors seeking to do business with the city. The document outlines three mandatory certifications that contractors must complete based on contract value thresholds: for contracts valued at $1,000 or greater, contractors must certify they do not boycott Israel; for contracts valued at $25,000 or greater, contractors must certify they do not knowingly employ illegal immigrants; and for contracts valued at $75,000 or greater, contractors must certify they do not boycott energy, fossil fuel, firearms, or ammunition industries.
The form requires contractors to check boxes confirming compliance with all three restrictions and provide their business name, City of Little Rock supplier/vendor number, signature, and date. The certifications are mandated by Arkansas state law under various sections of the Arkansas Code Annotated, and contractors must maintain compliance throughout the entire contract term. If circumstances change during the contract period that would affect any of these certifications, the contractor is required to notify the city in writing.
