About this file
This is a Bidder Certification of Bid Submission form issued by the Incorporated County of Los Alamos Procurement Division for the Piedra Loop Electric Replacement Project in New Mexico. The project involves the installation of conduit for primary electric and fiber, junction boxes, and flush mount pedestals along approximately 6,100 feet of Piedra Loop and adjacent side streets including La Rosa Court, Agila, and Mariposa Court. The work includes a riser installation at power pole #7076 for electric and fiber service, with the project beginning at the intersection of Piedra Drive and Piedra Loop and proceeding southeast. The contractor must coordinate transformer access with the utility company as the electric conduit will enter and exit existing transformers. The bidder must certify possession of all required New Mexico Contractor's License Numbers and Classifications necessary to fully perform the work.
Bidders must submit the certification form with their bid submission and are required to obtain a Los Alamos County Business License to perform work in the county. The bid amount must exclude state gross receipts tax and local option tax but include all other costs of doing business such as bonds, insurance, and profit, with applicable taxes shown separately on billing documents. The Incorporated County of Los Alamos reserves the right to reduce or add quantities during contract performance. Bidders must provide their Federal Tax I.D. Number, New Mexico CRS number if applicable, and any applicable N.M. Preference Certification. The organization's legal name, mailing address, physical address, telephone number, and fax number must accompany the certification, along with the signature and printed name of an authorized agent. All bidders certify non-discrimination policies regarding color, national origin, sex, religion, age, and disabled status in employment and service provision.
