About this file
This document is a Respondent Questionnaire (Attachment A) for a Request for Quote issued by the City of Detroit Office of Contracting and Procurement (OCP) on behalf of the General Services Department seeking qualified respondents to provide Fire Apparatus Maintenance and Repair Services for engine and components, including labor and/or parts. The contract term is three (3) years. The questionnaire requires respondents to provide comprehensive organizational and operational information, including single points of contact, firm background details, federal employer identification numbers, years in business, current employee counts, and prior experience with the City of Detroit. Respondents must document at least three (3) relevant experiences from the last five (5) years demonstrating their capacity to manage contracts of similar size and scope, and must provide detailed references including company names, project titles, client locations, contact information, dates of service, and descriptions of services provided that align with the RFQ requirements.
The questionnaire further requires respondents to disclose information regarding subcontractors, including their legal names, business addresses, percentage of work allocation, and relevant prior experience. Respondents must confirm that proposed subcontractors are not on the City of Detroit's debarred and suspended suppliers list. Additionally, the document requests organizational capacity documentation such as organizational charts and resumes for key personnel, employee dedication levels, and relevant experience qualifications. Respondents must identify any claims or lawsuits brought against their organization within the last ten (10) years, any prior contract terminations, and disclose any individuals or entities with personal or financial interest who have been debarred or suspended. The questionnaire also inquires whether respondents' proposed solutions involve artificial intelligence and requests information regarding Detroit resident hiring commitments and participation in Priority Hire Agreements with Detroit at Work.
