About this file

This is an Anti-Human Trafficking Affidavit form required by Broward County, Florida for vendors seeking to contract with the county. The affidavit certifies that the vendor does not use coercion for labor or services as defined under Florida Statutes Section 787.06. The form must be completed and returned with the vendor's submittal for the Broward County Landfill Split AC Units contract, which involves the removal, replacement, and installation of split air conditioning units for the Solid Waste and Recycling Services Department. If the affidavit is not provided at initial submittal, vendors must submit it within three business days of the county's request and upon contract award, renewal, or extension. Vendors who fail to comply within the stated timeframe may be deemed nonresponsive.

The affidavit requires the vendor to provide their company name, authorized representative signature, printed name, title, and date of execution under penalties of perjury. The form was last updated on September 9, 2024. All vendors must electronically submit their solicitation response through BPRO (Broward Procurement Requirements Online) by the closing date and time, with the affidavit being a mandatory component of a complete submission. The county reserves the right to request this form from vendors who do not initially provide it, with a three-business-day compliance deadline to avoid nonresponsive status determination.
