About this file
This is an Anti-Human Trafficking Affidavit form required by Broward County, Florida for vendor submissions related to the ChargePoint DC Chargers Repair and Maintenance Service contract. The affidavit must be completed and returned with the vendor's initial submittal, or if not provided, the vendor must submit the form within three business days of the County's request and upon contract award, renewal, or extension. Vendors are required to certify under penalties of perjury that they do not use coercion for labor or services as defined in Section 787.06, Florida Statutes. The form requires the vendor to provide their business name, authorized representative's signature, printed name, title, and date of execution. Failure to fully comply with the submission requirements within the stated timeframe may result in the vendor being deemed nonresponsive.
This affidavit is a mandatory compliance document rather than a pricing or technical submission element. It serves as a pre-award certification requirement for all vendors seeking to contract with Broward County, demonstrating the vendor's commitment to lawful labor practices and compliance with Florida anti-trafficking statutes. The form does not contain pricing information, set-asides, funding sources, or contract term details, as its sole purpose is to document the vendor's attestation regarding human trafficking compliance prior to contract execution.
