About this file

This document is an Acknowledgements & Preference Application form for a Louisville Metro Government contract opportunity. The form requires vendors to acknowledge receipt of any addenda to the proposal and provides options for two preference certifications: a Minimum Wage Preference and a Local Vendor Preference.

The Minimum Wage Preference section allows vendors to certify that they pay all full-time employees at least $10.10 per hour, with a potential penalty of 30% of the contract amount if false information is provided. Similarly, the Local Vendor Preference section enables vendors to claim local vendor status as described in Section I of the proposal, with a potential fine of 25% of the total contract if false information is submitted. Both preference options require vendors to check either "Yes" or "No" to indicate their eligibility and willingness to be considered under these provisions.
