About this file

This is a Limited Asbestos Inspection and Lead-Based Paint Survey report prepared by Hill West Environmental for the State of Utah Department of Administrative Services, Division of Facilities Construction and Management (DFCM), regarding mechanical upgrades at the Ogden Driver's License Division located at 615 East 5300 South, Ogden, Utah 84405. The inspection was conducted on June 9, 2026, by certified inspector Claude Dahlk (CHMM, CIEC, ASB-0433) to identify asbestos-containing materials (ACM) and lead-based paint components that would be impacted during the planned renovation. The survey focused exclusively on areas affected by the mechanical upgrade project, including roof core and black baseboard materials. Six samples were collected and analyzed using polarized light microscopy (PLM) testing by Eurofins Aerotech Built Environment Testing, LLC, with analytical results received on June 16, 2026.

The inspection findings determined that no asbestos-containing materials were detected in any of the six samples collected, and no lead-based paint materials would be impacted during the project, therefore no lead samples were collected. Additionally, no universal, hazardous, or toxic wastes were identified that would be impacted during the renovation activities. The report specifies that contractors must cease work and notify DFCM immediately if they discover suspect materials not included in this survey. This limited inspection was conducted in compliance with Utah Division of Air Quality, Utah Occupational Safety and Health Administration (UOSHA), and Salt Lake Valley Health Department (SLVHD) regulations. The scope is limited to the specific renovation areas and does not represent an exhaustive hazard assessment of the entire facility.

EXECUTIVE SUMMARY

On June 9, 2026, Hill West Environmental conducted a limited pre-renovation inspection for asbestos and lead-based paint at Ogden Driver’s License Division located at 615 East 5300 South, Ogden, Utah 84405. The project includes mechanical upgrades of the Ogden Driver’s License Division.

Ms. Aubrey Stapel, Project Manager with the State of Utah, Division of Facilities Construction and Management (DFCM) requested this inspection to identify asbestos-containing materials (ACM) and building components with lead-based paint that need to be addressed before the building is renovated.

Suspect materials that will be impacted included roof core and black baseboard.

Areas in which renovations are to occur were inspected, all other areas were not inspected as part of this survey. Hill West identified the following asbestos-containing materials (ACM) and lead-based paint during this inspection:

Asbestos-Containing Materials

Homogeneous Area Number

Cost Estimate

N/A No asbestos materials were identified in the samples collected during this inspection.

No Universal, Hazardous, and Toxic Wastes will be impacted during this renovation project. N/A

Conclusions

ACM – Asbestos Containing Materials (ACM) were not detected within any of the suspect building materials that were tested within the project area.

If the renovation or demolition contractor discovers additional suspect building materials that were not sampled as part of this inspection, they shall stop working and notify DFCM so that the materials can be sampled.

Lead – Lead materials will not be impacted during this renovation project and therefore, no samples were collected. If the contractor discovers other materials that will be impacted during this project that were not sampled, they shall stop work and alert DFCM that suspected materials require additional testing to determine if they are regulated.

Universal, Hazardous and Toxic Wastes – No Universal, Hazardous, and Toxic Wastes will be impacted during this renovation project.

1.0 INTRODUCTION

As per DFCM’s instructions, Hill West Environmental conducted a limited asbestos inspection of the Ogden Driver’s License Division located at 615 East 5300 South, Ogden, Utah 84405. Due to the remodel and upgrade projects, Hill West was instructed to only inspect areas that will be impacted during the renovation project.

The purpose of this survey was to identify the existence, extent, and condition of both friable and non-friable asbestos-containing materials (ACM) and lead-based paint within the renovation areas.

2.0 SITE VISIT /OBSERVATIONS

2.1 Asbestos Inspection

On June 9, 2026, Hill West Environmental visited the site to survey the renovation areas of the building. A complete asbestos inspection of the areas that will be impacted during renovations was made to determine conditions and develop a sampling scheme for the areas. Table 1 summarizes the asbestos inspection results.

4.0 DISCUSSION/CONCLUSTIONS

In Utah, EPA asbestos regulations are administered by the Utah Division of Air Quality. Utah Occupational Safety and Health Administration (UOSHA) has adopted the Federal OSHA regulations. In addition, the Salt Lake Valley Health Department (SLVHD) regulates demolition activities in Salt Lake County. The SLVHD regulations for pre-demolition building inspections require an asbestos inspection, but also require building owners to inspect the building for other hazardous materials such as universal wastes, hazardous and toxic wastes and lead-based paint. Similar to asbestos, these wastes, if present must be removed prior to demolition/renovation.

A copy of this report should be on-site during all renovation activities.

This limited asbestos and lead-based paint inspection was conducted at the Ogden Driver’s License Division that will be impacted during renovation activities.

Only items that are believed to be impacted during renovation activities were inspected during this survey. This survey is not intended to represent an exhaustive research into all asbestos or other hazards or conditions which may exist within other buildings located at this project site.

This report does not purport to represent future indoor conditions or events.

Situations or activities which transpire subsequent to this report, which result in adverse environmental, construction and/or engineering impacts, are not to be construed as relevant to this study.

This report is intended for the sole use of Hill West's client. This report may not be used or relied upon by any other party without the written consent of Hill West.

The scope of services performed in execution of the evaluation may not be appropriate to satisfy the needs of other users, and the use or re-use of this document or the findings, conclusions, or recommendations is at risk of said user. The results include an inherent uncertainty of measurement associated with estimating percentages by polarized light microscopy. Measurement uncertainty data for sample results with >1% asbestos concentration can be provided when requested.

The test report shall not be reproduced except in full, without written approval of the laboratory. The report must not be used by the client to claim product certification, approval, or endorsement by NVLAP, NIST, or any agency of the federal government. The Company reserves the right to dispose of all samples after a period of thirty (30) days, according to all state and federal guidelines, unless otherwise specified.

All components not quantified as asbestos content and non-asbestos content are considered to be non-fibrous matrix components. Matrix components may include, but are not limited to, gypsum, paint, silicate minerals, vinyl, binder, calcium carbonate, tar, and foam.

Inhomogeneous samples are separated into homogeneous subsamples and analyzed individually. ND means no fibers were detected. When detected, the minimum detection and reporting limit is less than 1% unless point counting is performed. Floor tile samples may contain large amounts of interference material and it is recommended that the sample be analyzed by gravimetric point count analysis to lower the detection limit and to aid in asbestos identification.
