About this file

This is a Service Level Agreement (SLA) document for rubbish removal and recycling services in Michigan, establishing performance standards and accountability measures for the contractor. The agreement outlines mandatory service requirements across multiple service locations, with performance measured separately for each location. The contractor must maintain 100% compliance across all SLA categories, including scheduled pickup completion, service request response, container repair and replacement, timely reporting, invoice accuracy, same-day compactor return for Michigan Department of Corrections (MDOC) facilities, site cleanliness, and service implementation and changes. Response times vary by service type, ranging from four business hours for complaint acknowledgment to three business days for billing inquiry resolution. The contractor is required to correct missed pickups within one business day of notification and must provide temporary replacement equipment when needed.

The agreement establishes a comprehensive service credit system as the primary enforcement mechanism, with penalties ranging from $25 to $500 per occurrence depending on the violation type and severity. Missed pickups incur $25 credits for the first instance and $50 per day for each subsequent day unresolved; after a second missed pickup at the same location within a month, the State reserves the right to procure services from a third party. Inaccurate invoices require correction within three business days, with $250 credits assessed after the third inaccuracy in a calendar year for each additional inaccuracy. The State may apply service credits against contractor invoices or require direct payment to the State within ten days of notice. Any unauthorized service interruption results in a $500 credit plus $250 for each additional day of suspension. The contractor must submit monthly reports by the sixteenth calendar day following month-end, with $100 credits assessed initially and every five business days thereafter for late or incomplete reports.
