About this file
This document is a Bidder Declaration form issued by the California Department of General Services, Procurement Division, for use in responding to Invitation for Bid (IFB) 26-3044 for Daily Courier Services with the California Department of Social Services, Disability Determination Service Division (DDSD) Oakland Branch. The form requires all prime bidders to complete mandatory declarations regarding their business certifications and subcontracting arrangements. Prime bidders must identify any current State of California certifications, including Microbusiness (MB), Small Business (SB), Nonprofit Veteran Service Agency (NVSA), or Disabled Veteran Business Enterprise (DVBE) status. Bidders must disclose whether subcontractors will be used and, if so, must clearly delineate the distinct elements of work to be performed by the prime bidder versus subcontractors.
The form establishes specific requirements for certified bidders to ensure compliance with state procurement regulations. All certified bidders, whether MB, SB, NVSA, or DVBE, must provide a commercially useful function as defined in applicable Military and Veterans Code and Government Code sections, or their bids will be deemed non-responsive and rejected. For subcontractors, bidders must complete a detailed table providing subcontractor names, contact information, addresses, California certifications, work descriptions, percentage of bid price, good standing verification, and rental equipment ownership percentages where applicable. DVBE prime bidders must additionally declare whether they are acting as a broker or agent and must confirm ownership of at least 51 percent of rental equipment if applicable. The form emphasizes that failure to properly complete this declaration or meet the requirements for commercially useful function will result in bid rejection.
