About this file

This document is a Past Performance Questionnaire template issued by the U.S. Government Publishing Office (GPO) Acquisition Services for use in federal contract proposals.

The questionnaire is completed in two sections: contractors complete fields capturing contract identification information (contractor name and address, contract number, contract date, period of performance, place of performance, completion date, contract value, project name, and type of contract), along with a description of the project's key characteristics. The evaluating organization representative completes the assessment section, providing their name, address, position during project execution (Project Manager, Contracting Officer's Representative, or other), phone number, and the number of months they monitored the contractor's performance. The evaluator then rates the contractor across nine performance categories using a standardized five-point scale: Exceptional (EX), Very Good (VG), Satisfactory (S), Marginal (MG), Unsatisfactory (US), or Not Observed (N/O). These categories include quality of products and services, overall performance, schedule adherence, technical requirements fulfillment, cost control, subcontracting compliance, customer satisfaction in routine matters, customer satisfaction during unexpected difficulties, and likelihood of repeat business. The form also requires narrative responses identifying the contractor's weak points, strong points, other completed projects, and average award fee percentage if applicable. Completed questionnaires must be submitted to Contracting Officer Troy White (twhite@gpo.gov) and Contract Specialist Michaela Farmer (mfarmer@gpo.gov). The information is classified as source selection sensitive under 41 U.S.C. 423.
