About this file
The Affirmation and Disclosure Form is a state contract compliance document requiring contractors to disclose specific details about their business operations and service locations. The form mandates that contractors affirm they have read and understand executive orders prohibiting offshore services, locating state data offshore, or purchasing from Russian institutions. Contractors must provide comprehensive details about their principal business location, service performance locations, and data storage sites, including information for any subcontractors involved in the contract.
The document emphasizes ongoing transparency, requiring contractors to immediately notify the state of any changes in service or data location before, during, and after contract execution. Failure to provide complete location information may result in contract non-award. The state retains the right to immediately terminate the contract if services are performed outside the United States without a signed waiver. The form must be signed by an authorized contractor representative, acknowledging understanding that this disclosure is an integral part of any potential state contract.
