About this file
This is a Past Performance Questionnaire form designed to evaluate contractor experience and qualifications for the NLS DTBM Breath Switch solicitation. The questionnaire template is intended to gather information from prospective contractors regarding their relevant past performance on similar projects, technical capabilities, and project management experience. Respondents are expected to provide detailed information about previous contracts, including project scope, technical accomplishments, schedule performance, cost management, and quality metrics that demonstrate their ability to successfully execute the non-recurring engineering (NRE) and full-production phases of the breath switch development contract. The form serves as an evaluation criterion for this simplified acquisition IDIQ contract with the Library of Congress, allowing the government to assess contractor capability to design, develop, and manufacture the breath switch device with its associated microcontroller, circuitry, USB connector, housing, food-safe silicone tubing, and required technical data package.
