Statement of Work Department of Homeland Security (DHS)
Federal Emergency Management Agency (FEMA)
1.0 PROJECT TITLE: Vault Door Replacement
2.0 SPECIFIC TASKS
• Contractor will remove existing vault door and frame and install one new Overly Model No. C5V-IIL-K/Class 5 High Security Vault door and frame
• Contractor will ensure all work to be conducted and to be performed in accordance with engineering standards.
2.1 Scope of Work
1. Site Preparation
• Contractor will perform site assessment and verify dimensions of existing vault door
• Contractor will coordinate with facility management to schedule downtime and access
• Contractor will protect surrounding area with appropriate coverings and signage to prevent damage and ensure safety
2. Removal of Existing Vault Door
• Contractor will safely remove the current vault door, frame, and associated hardware
• Contractor will dispose of old door and materials in accordance with local regulations and security protocols
3. Installation of New Vault Door
• Contractor will install new vault door as specified by manufacturer and project requirements
• Install new frame, hardware, lock mechanisms and access control
• Ensure door is properly aligned, operates smoothly, and is secure
4. Cleanup and final inspection
• Contractor will clean work and remove debris and materials
• Contractor will conduct final inspection with facility representative for sign-off
3.0 DELIVERABLES/INSPECTION SCHEDULE
3.1 Mechanical Work Inspection and Approval:
• All specifications, configuration, and plans for the replacement of vault door will be approved by the COR prior to start of the project.
• All mechanical work including modifications will be approved by the COR.
• After this section has been completed, the COR will inspect the work.
3.2 COR acceptance and final Inspection of all work to be performed:
• The COR and other management personnel from the MERS Detachment will conduct acceptance inspections of all construction work and materials.
FEMA personnel will be authorized to perform scheduled and un-scheduled inspection of the construction work to oversee the progress.
4.0 PLACE OF PERFORMANCE
• The contractor will perform the work at the FEMA, MERS, VSAB, Bldg.
B, located at 65 Old Marlboro Road, Maynard, MA 01754
5.0 SECURITY
• Contractor will submit the names of their prime and sub-contractors to the COR before the start of the project so that they can be cleared by FEMA Security to work on the premises.
6.0 PERIOD OF PERFORMANCE
• The period of performance for this award shall not exceed 180 calendar days from contract award date.
