About this file
This is an Attachment F document to Request for Proposal S-44300-00017234 issued by the State of Oregon, acting through the Oregon Health Authority's Child, Family, Lifespan Behavioral Health Program, for the development and operation of the Oregon Fidelity Wraparound Center for Excellence (FWC4E). The attachment outlines the Certified Disadvantaged Business Outreach Plan requirements that proposers must complete as part of their RFP submission. The plan requires proposers to identify their certification status under ORS 200.055 by the Oregon Certification Office for Business Inclusion and Diversity (COBID), including designations as disadvantaged businesses (DBE), minority-owned businesses (MBE), woman-owned businesses (WBE), service-disabled veteran-owned businesses (SDV), or emerging small businesses (ESB). Proposers must list certified firms with which they have had contractual relationships within the past two years and identify any non-certified firms that may be minority-owned, woman-owned, service-disabled veteran-owned, or emerging small businesses.
Proposers who foresee subcontracting opportunities must complete the full Certified Disadvantaged Business Outreach Plan, which requires them to describe the steps they will take to solicit certified firms for subcontracting, identify specific subcontracting opportunities with approximate dollar values, and confirm their willingness to report subcontractor identity and subcontract values to COBID if awarded the contract. The document certifies that proposers will take reasonable steps to ensure that certified firms are provided equal opportunity to compete for and participate in subcontract performance on any contract resulting from this procurement. The information submitted in response to this outreach plan requirement will not be considered in any scored evaluation of the proposal.
