About this file

This is an Organizational Conflict of Interest (OCI) Disclosure Form required for the Request for Project Proposal (RPP) Number W91QEY-26-R-A002 issued by the U.S. Army.

The form mandates that all proposing offerors—whether serving as prime offerors, joint venture partners, or proposed subcontractors—formally disclose any actual, potential, or apparent conflicts of interest. Offerors must indicate whether they have received assistance, advice, proprietary data, or non-public information from current or former federal employees (military or civilian) or government contractors regarding this effort. The form provides three disclosure options: Option A for negative responses indicating no conflict; Option B for negative responses where an Agreements Officer has determined the OCI has been mitigated with supporting documentation; and Option C for disclosures of actual, potential, or apparent conflicts. When Option C is selected, offerors must complete Section III with detailed information including the source of information or assistance, identifying information about the individual or entity involved, and a comprehensive description of the nature of interaction and information provided, including dates and context. The completed form must be submitted as an attachment to the RPP package, and failure to provide accurate disclosure or submission of false or misleading information may result in disqualification from award consideration. Compliance with this disclosure requirement is identified as a material requirement of the solicitation.
