About this file
This is a Bid Cover Sheet for Invitation to Bid (ITB) #73000-00018201 issued by the Oregon Department of Transportation, Fleet Operations and Services. The solicitation seeks to purchase three Schulte FX-318, 18' Flex Wing Rotary mowers or approved equivalent units. The cover sheet serves as the standardized bidding document that requires submission of bidder information, qualifications, certifications, and references for comparable projects of similar size and scope. Bidders must complete all sections of the form and certify acceptance of the ITB terms, conditions, and sample agreement, with pricing to remain effective for a minimum of one year following award.
The bid cover sheet establishes certification and compliance requirements for all bidders, including confirmation that bidders are not debarred or suspended from federal or state contracting, have no active exclusions in the System for Award Management (SAM), and comply with Oregon Revised Statutes regarding nondiscrimination in employment and equal opportunity practices. Bidders must also disclose any conflicts of interest per ODOT guidelines, certify tax compliance, and for anticipated contract prices of $150,000 or more, provide evidence of written policies preventing sexual harassment and discrimination. The document includes provisions for the Buy Oregon Preference, allowing bidders to claim preference status for goods fabricated or processed entirely within Oregon in accordance with ORS 279A.128. Bidders must submit three professional references with detailed contact information and project descriptions demonstrating experience on comparable contracts.
