About this file

This is a Proposal Cover Sheet for a Mini-RFP issued by the Oregon Department of Transportation (ODOT) for the Willamette Falls Drive: 16th Street – Ostman Road Pedestrian/Bicycle Upgrades project (Requisition #S-73000-00017305, Project #23242). The document serves as the formal submission cover for proposals responding to this architecture and engineering (A&E) full-service solicitation. Proposing firms must identify their primary contact information and certify compliance with all minimum requirements, including engagement of an Oregon Registered Professional Engineer and an Oregon Registered Professional Land Surveyor to perform services under the Work Order Contract (WOC). The cover sheet requires proposers to provide registration information and credentials for the designated engineers and surveyors who will be performing work on this project.

The document establishes mandatory certifications that proposers must attest to upon submission, including receipt of all addenda, truthfulness of proposal contents, non-discrimination commitments, and compliance with Oregon's sexual harassment and discrimination prevention policies (applicable for contracts with anticipated prices of $150,000 or more). Proposers must confirm they have no undisclosed liquidated and delinquent debt owed to the State of Oregon and must complete Conflict of Interest (COI) disclosures as required by ODOT guidelines. The authorized representative of the proposing firm must sign and date the cover sheet to certify their authority to bind the organization and make all representations contained in the proposal. Pricing information, specific evaluation criteria, and detailed project scope are contained in the referenced Mini-RFP document rather than in this cover sheet.
