About this file

This is a Proposer Information and Certification Sheet (Attachment C) for RFP/Bid Solicitation 73000-00017224 issued by the State of Oregon for Region 1 On-Call Graphics Support Services. The document serves as the standardized form that proposers must complete and submit as part of their bid response. It requires proposers to provide basic organizational information including legal name, address, state of incorporation, entity type, and primary contact details. Proposers must also acknowledge receipt of all RFP addenda and confirm that their proposal constitutes a firm offer valid for 180 days following the bid opening date as identified in OregonBuys. Upon award, proposers agree to perform the specified scope of work and meet all performance standards as outlined in the final negotiated Price Agreement.

The certification sheet contains mandatory attestations covering compliance with Oregon state laws and regulations, including tax compliance, non-discrimination in employment and subcontracting practices, wage and hour requirements, broadband service compliance, and debarment status verification. Proposers must certify absence of conflicts of interest per ODOT guidelines and confirm that all proposal contents are truthful and prepared independently without collusion or fraud. An authorized representative must execute the certification by entering their name, title, and date while checking the "APPROVED" checkbox, thereby binding the proposer to all representations and certifications. The document explicitly references that these certifications are supplementary to any additional certifications required in the Price Agreement and Statement of Work detailed in Attachment A.

As the authorized representative named below, I certify on behalf of Proposer that:

Proposer acknowledges receipt of any and all Addenda to this RFP.

Proposal is a Firm Offer for 180 days following the Bid Opening Date identified in OregonBuys.

If applicable, Proposer has, or will have prior to contract execution, a written policy and practice, that meets the requirements described in ORS 279A.112 (formerly HB 3060), of preventing sexual harassment, sexual assault and discrimination against employees who are members of a protected class. (Agency may not enter into a contract with an anticipated contract price of $150,000 or more with a Proposer that does not certify it has such a policy and practice. See https://www.oregon.gov/DAS/Procurement/Pages/hb3060.aspx for additional information and sample policy template.)

Proposer complies with ORS 652.220 and does not unlawfully discriminate against any of Proposer’s employees in the payment of wages or other compensation for work of comparable character on the basis of an employee’s membership in a protected class. “Protected class” means a group of persons distinguished by race, color, religion, sex, sexual orientation, national origin, marital status, veteran status, disability or age.
