About this file
This is a Proposal Cover Sheet for a Mini-RFP solicitation issued by the Oregon Department of Transportation (ODOT) for the Wasco County Transportation System Plan (TSP) Update, a Transportation and Growth Management (TGM) project with file code 4B-24 and solicitation number S-73000-00016025. The cover sheet serves as the required submission document for interested firms to respond to this opportunity. Proposers must complete the form with their firm name, primary contact information, and documentation of how they meet any minimum requirements specified in the Mini-RFP. The sheet requires identification of any proposed subconsultants and their respective disciplines or work assignments. This cover sheet does not specify response deadlines, site visits, bidder meetings, or contract award dates, as those details would be contained in the primary Mini-RFP document itself.
The cover sheet requires proposers to certify compliance with multiple statutory and policy requirements, including Oregon Revised Statutes regarding non-discrimination in subcontracting, sexual harassment and discrimination prevention policies, and conflict of interest disclosure obligations. Proposers must confirm they have received all addenda, accept all terms and conditions of the Mini-RFP, have no undisclosed liquidated debt owed to the State, and certify the truthfulness and accuracy of their proposal contents. An authorized representative must execute the cover sheet by entering their name, title, and date while checking the "APPROVED" checkbox to bind the proposing firm. The cover sheet does not address pricing, funding sources, or set-asides, indicating these elements are detailed in accompanying solicitation documents.
