About this file

This document is a Proposal Cover Sheet for a mini-solicitation (Mini-RFP #S-73000-00013572) issued by the Oregon Department of Transportation (ODOT) for a transportation infrastructure project involving curb ramps in Happy Valley and Mulino, Oregon, specifically on OR224 and OR213. The cover sheet serves as a formal certification document where the proposing firm attests to various legal and procedural compliance requirements.

The cover sheet includes key certifications that the proposing firm must complete, such as acknowledging receipt of addenda, agreeing to non-discrimination policies, having a sexual harassment prevention policy, confirming no outstanding state debt, accepting all mini-RFP terms, and making required conflict of interest disclosures. Notably, for contracts anticipated to be $150,000 or more, the proposer must have a written policy preventing sexual harassment and discrimination, and must disclose any potential conflicts of interest involving current or former ODOT employees within the past two years.
