About this file

This is Addendum No. 1 to contract documents for the State of California Department of State Hospitals, issued by the Administrative Services Division, Business Management Branch, Facilities, Planning and Construction Management. The addendum pertains to the West Perimeter Bridge project at Patton State Hospital in San Bernardino County, California (Project No. 26-30612-000). The project involves the replacement of the existing West Perimeter Vehicle Bridge, including demolition and removal of the existing bridge structure, installation of a new vehicle bridge, construction of ADA-compliant warning curbs, removal and relocation or protection of existing water lines, and reconstruction of pavement and approach transitions on both sides of the bridge. The addendum was issued on September 15, 2026, with a bid due date of October 20, 2026.

The addendum modifies Section 00 73 00 (Supplementary Conditions) of the contract documents, specifically removing the original work phase structure and replacing it with revised phasing requirements. Work shall be executed in two phases: Phase 1 consists of rough-in work (electrical, low voltage, and plumbing) released on a unit-by-unit basis with coordination through Hospital Staff and the Construction Manager, and Phase 2 encompasses demolition and modification of medication dispensing rooms following Phase 1 completion and sign-off for each unit. The contract time is 365 calendar days from the Notice to Proceed, with liquidated damages of $3,000 per calendar day for delays beyond the contract period. The State will withhold liquidated damages from monthly payments if the Contractor falls more than 14 calendar days behind schedule. Contractors are required to notify the State immediately of any changes to name, address, Federal Employer Identification Number, legal status, ownership, officers or key personnel, licenses, or other material information, with updated Standard Form 204 documentation required within five calendar days.
