AWARD PURPOSE THE PURPOSE OF THE JUVENILE TRANSITION PROGRAM IS TO PROVIDE AT-RISK YOUTH OPPORTUNITIES TO GAIN SKILLS AND POSITIVE BEHAVIORS IN ORDER OBTAIN EMPLOYMENT AND TO PREVENT OFFENDING OR RECIDIVATING. ACTIVITIES PERFORMED MMGI PROPOSES TO SERVE 160 HIGH-RISK YOUTH IN TARGET COMMUNITIES OF ST. LOUIS CITY AND COUNTY THROUGH OUR JUVENILE TRANSITION PROGRAM. WORKING WITH THE COURTS AND OUR OTHER JUSTICE PARTNERS TO PROVIDE OUTREACH, WE WILL RECRUIT ELIGIBLE YOUTH FOR THE DOL GROWTH OPPORTUNITIES GRANT PROGRAM. UPON RECRUITMENT, OUR DIRECTOR WILL ASSIGN A CASE MANAGER, WHO WILL OPEN A FILE ON THE PARTICIPANT AND SCHEDULE INTAKE. THE CASE MANAGER WILL BE RESPONSIBLE FOR VERIFYING THE YOUTH'S ELIGIBILITY, COORDINATING SERVICES, AND ENSURING PARTICIPANTS PROGRESS THROUGH THE VARIOUS ASPECTS OF THEIR SERVICE PLAN. SERVICES WILL INCLUDE, AS APPROPRIATE, NEEDS ASSESSMENT, EMPLOYMENT BARRIER IDENTIFICATION, REFERRAL FOR MENTORING AND VIOLENCE PREVENTION SERVICES AND OTHER COMMUNITY-BASED SERVICES, CAREER EXPLORATION AND PLANNING, JOB PREPARATION, TRAUMA-INFORMED CASE MANAGEMENT (TO INCLUDE FOLLOW-ALONG), WORK EXPERIENCE, AND TRAINING. DELIVERABLES WIOA PRIMARY INDICATORS OF PERFORMANCE: EDUCATION & EMPLOYMENT RATE - SECOND QUARTER AFTER EXIT 70% EDUCATION & EMPLOYMENT RATE - FOURTH QUARTER AFTER EXIT 60% MEDIAN EARNINGS - SECOND QUARTER AFTER EXIT $5,750 CREDENTIAL ATTAINMENT 50% MEASURABLE SKILLS GAIN 70% REO-SPECIFIC INDICATORS: ARRESTS FOR VIOLENT CRIMES THAT ARE COMMITTED AFTER PROGRAM ENTRY: THE GOAL IS LESS THAN 5%.
