Description
Integrated mobile & safe space prevention at apnh - apnh: a place to nourish your health has a successful record of providing HIV/aids and prevention services to infected and affected individuals in the greater new haven, connecticut area. We effectively engage clients who are from the most at-risk populations. This is evident when analyzing who has accessed our HIV prevention services in the last 24 months: 54% of clients are 13 to 29 years of age, 76% of clients are male, 65% of clients identified as non-white (with 60% identifying as black/african american or latinx/hispanic), and 56% of clients identifying as men who have SEX with men (MSM). As leaders in reaching highly impacted populations, we plan to expand our impact in the community by focusing efforts on young, black/latino MSM. The strategies and activities proposed in this grant will allow apnh to improve service to black/latino MSM in the greater new haven area as outlined by the ending the epidemic initiative pertaining to diagnosing, treating, preventing, and responding to HIV infections. Using funds acquired from PS21-2102, our impact in the area will become more profound with the development of integrated testing services, the establishment of a safe space for our focus populations, and the utilization of a mobile testing unit. Over the course of this project period, these services will all integrate to increase our testing capacity by 60%, link all newly diagnosed individuals into care, provide integrated STD screenings, increase access to prep, provide a safe space in the community for young, black and latino MSM, address health disparities related to HIV, increase the number of newly diagnosed individuals we identify to at least 8 persons a year, and to overall reduce new HIV infections in the greater new haven area.
