About this program
The Department of Health and Human Services Office of the National Coordinator for Health Information Technology (ONC) funds HL7 standards development and implementation activities to advance interoperability among health IT systems, under CFDA 93.826, with the most recent grant award dated September 24, 2020.
This program operates as a sole-source cooperative agreement mechanism with Health Level 7 International (HL7), an ANSI-accredited standards development organization. Funded activities include developing and publishing HL7 standards such as Consolidated Clinical Document Architecture (C-CDA) and Fast Healthcare Interoperability Resources (FHIR); enhancing infrastructure for implementing and using those standards; and empowering the HL7 standards development community. Recent work has focused on expanding standards to address public health emergency response gaps—specifically disease surveillance, situational awareness, logistics data (ICU bed and ventilator availability), privacy and security frameworks, API support for population-level services, and social determinants of health standards. Recipients can subgrant or contract with sub-recipients as outlined in funding notices.
The program has issued four awards over the past decade, with a relatively sparse cadence: one award per year in 2015, 2016, and 2017, then a gap until 2020. Combined federal dollars obligated total $8.3 million. The Office of the National Coordinator for Health Information Technology dominated recent funding, obligating $7.3 million in a single 2017 cooperative agreement to HL7 for C-CDA and FHIR maturation (award 90AX0019, ultimate completion September 2022). A 2020 cooperative agreement (90C30007) on public health standards for future pandemics shows $0 obligated but ran through February 2025. Earlier Project Grant awards addressed SMART Gallery infrastructure and Consolidated CDA implementation enhancement.
The assistance type mix is evenly split between Cooperative Agreements (50%) and Project Grants (50%). All beneficiaries include health care organizations and patients using electronic health records. Grant records must be retained for a minimum of three years per 2 CFR 200. No recent open solicitations are documented; the last posted opportunity (NAP-AX-22-002) closed August 19, 2022.
