About this program
The Department of Health and Human Services Centers for Disease Control and Prevention distributed $392.1 million through CFDA 93.724 to fund communities implementing population-based chronic disease prevention and wellness interventions, with the most recent grant awards made on September 30, 2010. This program, established under the American Recovery and Reinvestment Act of 2009, is no longer active—all 44 awards were disbursed in 2010 and have reached completion.
The program funded communities implementing evidence-based strategies to increase physical activity, improve nutrition including reducing salt and trans fats and increasing fruit and vegetable consumption, decrease smoking prevalence and teen smoking initiation, and reduce secondhand smoke exposure. Recipients deployed population-based policy, systems, and environmental changes across five evidence-based intervention strategies—access, media, point-of-decision information, social support services, and price—in both school and community settings. The cooperative agreements addressed Healthy People 2010 focus areas including obesity, cardiovascular disease, tobacco, and educational and community-based programs.
All 44 awards were structured as Cooperative Agreements, indicating active federal involvement in grant management. The CDC was the sole funding source across the entire portfolio. Award amounts ranged widely, from under $1 million to approximately $18 million per award; representative recipients included cooperative agreements U58DP002543 ($18.0 million for tobacco prevention and control), U58DP002485 ($17.7 million for Communities Putting Prevention to Work), and U58DP002496 ($17.9 million for Communities Putting Prevention to Work). Most awards carried project completion dates between March 2012 and March 2013.
No grant opportunities are currently open under this Assistance Listing. The program funded a single cohort in 2010 under ARRA appropriations and has not posted new solicitations since that time.
Objective and description
- Objective
This funding was appropriated under the American Recovery and Reinvestment Act of 2009 (Public Health Service Act, 42 U.S. Code 241(a) and 247b (k) 2) to reduce Chronic Disease risk factors, prevent and delay chronic disease, promote wellness, and better manage chronic conditions. This initiative will address the following: * Increased levels of physical activity * Improved nutrition (e.g. increased fruit/vegetable consumption, reduced salt and transfats); * Decreased smoking prevalence and decreased teen smoking initiation; and * Decreased exposure to secondhand smoke. Communities funded under this announcement will accomplish this by implementing population-based approaches such as policy, systems, and environmental changes across 5 evidence-based strategies - access, media, point of decision information, social support services, and price - in both schools and communities. Both components are necessary to achieve behavior change in youth that sustain from school settings into family and community settings, and supports sustaining healthy behavior into adulthood. Communities funded by this announcement will receive a menu of evidence-based interventions along with support and tools from the Centers for Disease Control and Prevention (CDC) to strengthen and develop effective strategies tailored to their needs. The cooperative agreements will address the "Healthy People 2010" focus areas of obesity, cardiovascular disease, tobacco, and educational and community-based programs; the Health Protection Goals of "Healthy People in Every Stage of Life" which states that "all people, and especially those at greater risk of health disparities, will achieve their optimal lifespan with the best quality of health in every stage of life," and "Healthy People in Healthy Places" which states that "the places where people live, work, learn, and play will protect and promote their health and safety, especially those at greater risk of health disparities."
