About this program

The Centers for Medicare and Medicaid Services is awarding cooperative agreements under the Section 206 Consolidated Appropriations Act, 2024 planning grant program (CFDA 93.694) to state Medicaid and CHIP agencies to develop operational systems and infrastructure supporting continuity of care for Medicaid and CHIP-eligible individuals following incarceration, with the most recent awards issued on August 11, 2025.

The program funds state-level planning and implementation activities addressing mandatory federal requirements under the Consolidated Appropriations Acts of 2023 and 2024. Eligible activities include designing pre- and post-release service delivery systems, building data-sharing infrastructure between correctional facilities and Medicaid agencies, developing electronic health record systems and health information exchanges, establishing Medicaid billing capacity within correctional providers, conducting statewide needs assessments, and implementing targeted case management and screening services. Awards support staffing, technical assistance, vendor contracts, IT systems development, interagency coordination, and community stakeholder engagement. The statutory mandate beginning January 1, 2026 prohibits termination of Medicaid and CHIP eligibility due to incarceration; the program supports operational readiness for this transition.

CMS is the sole funder of all 29 recent awards distributed across all states and territories, obligating $161.1 million in combined federal dollars. Award amounts range from approximately $1.6 million (Nebraska) to $13.1 million (Pennsylvania), with a median near $5 million over the four-year performance period through June 30, 2029. All awards are structured as cooperative agreements, reflecting CMS's ongoing involvement in implementation monitoring and technical guidance.

Recent awards demonstrate the scope: Pennsylvania's award funds electronic health record system implementation for eight youth development centers and camps; Indiana supports seamless Medicaid access for justice-involved youth ages 21 and under and former foster youth through age 26; Oklahoma is implementing an interoperable EHR system integrated with its offender management system; New Hampshire is purchasing telehealth booths for state prisons, county jails, and youth detention; North Dakota is building bidirectional data exchange between county and tribal jails and the state Medicaid system; and Virginia is onboarding staff to oversee continuity-of-care efforts statewide.

The grant opportunity (CMS-2T2-25-001) posted August 22, 2024 with a Cohort 2 due date of April 18, 2025 (now closed). No additional open solicitation window is currently documented in the source. All recent awards carry June 30, 2029 completion dates, indicating this as a four-year, fixed-term competitive funding cycle. Eligible applicants are state Medicaid and CHIP agencies only.

Description

CMS seeks applications for State Planning Grants for promoting continuity of care for individuals who are inmates of a public institution and are eligible for medical assistance under the state Medicaid program or the Children's Health Insurance Program (CHIP). Funds may be used for addressing operational barriers and improving systems for continuity of care following incarceration. These planning grants also work towards operationalizing new requirements which prohibit the termination of eligibility for Medicaid and CHIP due to incarceration for all Medicaid and CHIP enrollees beginning January 1, 2026.

Eligibility

Applicant eligibility

Only one application may be submitted for a given state or territory. The term "state Medicaid agency" means the single state agency for medical assistance provided under Title XIX and Title XXI of the Act while the term "state CHIP agency" means the state agency responsible for administering the state CHIP plan under Title XXI of the Act. Further, if a state administers their Medicaid and separate CHIP programs through separate agencies, both agencies must agree to work together and submit one application per state. A Tribal organization, if interested, must come under the auspices of and work with the state Medicaid or CHIP agency in the implementation of this initiative.

Uses of assistance

Funding is available for activities and expenses related to complying with the requirement that a state shall not terminate eligibility for medical assistance for individuals who are inmates of a public institution, for adopting Medicaid state plan options, or for promoting continuity of care following incarceration.

Length and time phasing of assistance

The period of performance for this planning grant is 4 years.
