About this opportunity
The Centers for Medicare & Medicaid Services (CMS) is seeking applications from eligible state Medicaid and CHIP agencies for planning grants to develop operational capabilities that promote continuity of care for individuals who are inmates of a public institution and eligible for medical assistance under Medicaid or CHIP. The grants aim to help states comply with new statutory requirements related to providing Medicaid and CHIP services to eligible juveniles surrounding their release from incarceration, as well as preventing termination of medical assistance for incarcerated individuals. Applicants must submit comprehensive documentation, including project narratives, work plans, budgets, and organizational capacity details. CMS will fund the selected applicants through cooperative agreements and provide substantial involvement during the four-year period of performance.
The total available funding for this opportunity is $106.5 million, with individual awards ranging from $1 million to $5 million per state. The grants will be awarded through two application cohorts, with the first cohort deadline on November 26, 2024, and the second cohort deadline on April 18, 2025. CMS expects to announce the Cohort 1 awards on February 15, 2025, and the Cohort 2 awards on July 15, 2025.
Synopsis
The Centers for Medicare & Medicaid Services (CMS) is seeking applications from eligible state Medicaid and CHIP agencies for planning grants to develop operational capabilities to promote continuity of care for individuals who meet both of the following criteria:
•Are inmates of a public institution, and
•Are eligible for medical assistance under the state Medicaid program or are eligible for child health assistance or pregnancy-related assistance under the state Children's Health Insurance Program (CHIP).
These planning grants also support states with operationalizing requirements under section 205 of division G of the CAA, 2024 (P.L. 118-42), as well as the CAA, 2023 (P.L. 117-328).
Funds may be used for addressing operational barriers and improving systems for continuity of care following incarceration in state-operated prisons, local, tribal, and county jails, and youth correctional or detention facilities.
CMS will fund cooperative agreements for a four-year period of performance.
