About this program

The Department of Health and Human Services Centers for Disease Control and Prevention (CDC) funds establishment and strengthening of Coordinated Chronic Disease Prevention and Health Promotion Programs within State and Territory Health Departments under Assistance Listing 93.544, with the most recent grant award disbursed on September 30, 2010.

The program supports development, implementation, and evaluation of chronic disease prevention initiatives addressing nutrition, physical activity and obesity, diabetes, heart disease and stroke, arthritis, and cancer prevention and control. Funded recipients establish or strengthen a Chronic Disease Prevention Unit to provide leadership and coordination across categorical programs, consolidate epidemiology and evaluation activities, foster inter-program collaboration, and identify population subgroups experiencing disproportionate disease burden.

All activity in the portfolio consists of a single Cooperative Agreement structure. The CDC obligated $13.3 million in total federal dollars across one award. The most recent (and only) grant award on record is Cooperative Agreement U58DP002005, awarded September 30, 2010 to fund the Georgia Healthy Communities Initiative and Georgia's Tobacco Use Prevention Program with $13.3 million obligated through August 28, 2014.

No grant awards have been disbursed since 2010, and no recent grant opportunities are posted. The program shows no activity within the past 16 years. The source does not indicate whether the Assistance Listing remains active or whether new solicitations are planned. Organizations considering pursuit under this program should verify current status with the CDC directly before allocating business development resources.

Objective and description

Objective

Coordinated Chronic Disease Prevention and Health Promotion Program - The purpose of the program is to establish or strengthen Chronic Disease Prevention Programs within State and Territory Health Departments, to provide leadership and coordination, and support development, implementation and evaluation of CDC funded programs, including those addressing nutrition, physical activity and obesity, diabetes, heart disease and stroke, arthritis, cancer prevention and control, and diabetes. These inter-related conditions share risk factors and intervention strategies that would benefit from coordinated, collaborative implementation with consolidated leadership and oversight to ensure achievement of outcomes, efficient use of resources, implementation of evidence-based interventions, and dissemination of best practices across programs. The award recipients will be expected to establish or strengthen a Chronic disease Prevention Unit that provides leadership for chronic disease prevention and control, establishes cross cutting policy, communications, epidemiology and evaluation activities with the Chronic Disease Prevention Unit to support an array of categorical programs, foster collaboration and efficient use of resources across existing or new categorical programs addressing chronic diseases and their associated risk factors, best position programs and resources to achieve population level change in proposed chronic disease health and risk factor outcomes, and to identify specific population subgroups that suffer disproportionately from the conditions or risk factors being addresses and narrow the gaps in health status between these population subgroups and the population as a whole.
