About this program
The Department of Health and Human Services Centers for Disease Control and Prevention funds community-based chronic disease prevention and health disparities reduction activities under CFDA 93.541, with the most recent grant award issued on September 30, 2010.
This program is inactive for new funding. The portfolio shows no grant awards since 2010, with the award cadence having declined sharply from 39 awards in 2007 to 3 in 2009 and 1 in 2010. No recent grant opportunities are currently open.
The program funded two main initiative tracks: Racial and Ethnic Approaches to Community Health (REACH) and Minority National Organizations for REACH (MNO REACH), both structured as cooperative agreements with state, territorial, and local health departments and national minority-focused organizations. Recipients designed, implemented, and evaluated community-driven strategies to eliminate health disparities and build capacity for chronic disease prevention in priority areas including breast and cervical cancer, cardiovascular disease, diabetes, adult immunization, hepatitis B, tuberculosis, asthma, and infant mortality. Work targeted African American, American Indian/Alaska Native, Asian, Native Hawaiian/Other Pacific Islander, and Hispanic/Latino populations. Awarded grants supported professional development, workforce training, and dissemination of evidence-based interventions.
All 43 awards in the portfolio are Cooperative Agreements, with combined federal dollars obligated of $96 million. The CDC was the sole funding agency. Award magnitudes ranged broadly: REACH cooperative agreements typically ranged from $1.5 million to $3.4 million per award, while the single 2010 REACH CORE award obligated $399,700. The 2009 Minority National Organizations for REACH awards each obligated approximately $1.25 million.
Pursuit of this program is not recommended given the absence of new funding activity for 16 years.
Objective and description
- Objective
The overall goals of this program are to reduce death and disability from leading causes of death through the prevention and control of these conditions and their risk factors. Racial and ethnic groups targeted include: African American/Black, American Indian/Alaska Native, Asian, Native Hawaiian/Other Pacific Islander, and Hispanic/Latino. Health priority areas include: breast and cervical cancer; cardiovascular disease; diabetes mellitus; adult/older adult immunization, hepatitis B, and/or tuberculosis; asthma; and infant mortality. REACH and Community Health program recipients will design, implement, evaluate, and disseminate community-driven strategies to eliminate health disparities as well as provide capacity-building to increase effectiveness of chronic disease prevention and health promotion programs at the state, territorial and local level through professional development and workforce training, identification, communication and use of effective evidence-based interventions. More specific goals within these broad goal areas will be provided in funding announcements.
