About this program

The Centers for Disease Control and Prevention awards Cooperative Agreements under the Racial and Ethnic Approaches to Community Health program (CFDA 93.304) to support implementation of culturally tailored, evidence-based strategies addressing chronic disease and health disparities in racial and ethnic minority populations; the most recent grant awards date to September 2014, making this program inactive for over a decade, though a fiscal year 2023 solicitation (CDC-RFA-DP-23-0014) was posted in August 2022 with an April 2023 due date.

The program funds community-based interventions targeting tobacco use reduction, improved nutrition, physical activity promotion, and immunization education, as well as linkages between community and clinical care systems. Recent awards demonstrate concrete focus: BE EPIC addressed health disparities among African Americans in Denver through policy and structural engagement; PROJECT RAISE supported healthy eating and active living among Asian Americans; STRIVE San Diego promoted healthy dining choices; and the REACH Program for Eastern California Tribes addressed disparities among tribal populations. Award magnitudes in the 2012–2014 cycle ranged from $300 thousand to $4.3 million, with most individual awards falling between $400 thousand and $500 thousand.

The program is structured overwhelmingly as Cooperative Agreements (96 percent of the 25 tracked awards), with 100 percent funding from CDC. Total federal dollars obligated across recent awards reached $29.6 million. The 2014 award cycle concentrated activity sharply, with 18 awards in that year alone compared to 7 in 2012.

Eligibility for the 2023 opportunity extends broadly to 501(c)(3) nonprofits, higher education institutions (public and private), state and local governments, tribal entities, and other entity types, provided applicants demonstrate a history of working with established community coalitions, base strategy selection on community health needs assessment, and possess organizational capacity for immediate implementation.

However, the program has not disbursed awards in over a decade. Despite the 2023 NOFO posting, no awards are evident in the dataset after September 2014. Applicants should verify current program status and posting of any new solicitation on Grants.gov before pursuing.
