About this program
The Department of Health and Human Services Administration for Community Living distributes $629.6 million in combined federal and non-federal funding through formula grants and block grants to states and territories for long-term care ombudsman services, with the most recent awards issued October 1, 2025.
This program funds state and territorial ombudsman operations that investigate and resolve complaints from residents of nursing homes, assisted living facilities, board and care facilities, and other long-term care communities. Ombudsmen work to protect residents' health, safety, welfare, and rights; provide information about long-term services and supports; ensure residents have regular and timely access to ombudsman services; represent resident interests before governmental agencies; and analyze and recommend changes to laws and regulations affecting long-term care facility residents.
Awards are distributed on a non-competitive basis through a statutory formula to State Agencies on Aging designated by governors in all 50 states and U.S. territories. The portfolio is overwhelmingly concentrated in formula grants (1,452 awards, 91% of the total) with a smaller share of block grants (138 awards, 9%). Recent annual award volume has stabilized at approximately 55–57 grants per year over the past four years. All funding originates from the Administration for Community Living within HHS.
Recent awards range from approximately $31 thousand (American Samoa, Mariana Islands, FY2026) to $5.2 million (California, FY2026). Representative recent FY2026 awards include Texas ($3.6 million), Florida ($3.9 million), New York ($3 million), Illinois ($1.8 million), and Ohio ($1.8 million). These are statutory grants with 2-, 3-, or 4-year performance periods; the FY2026 awards obligated on October 1, 2025 extend through September 30, 2027. Non-federal cost-share is minimal; the $630.2 million total obligated reflects the $629.6 million federal share plus only $600 thousand in non-federal contribution.
No discretionary grant opportunities are posted. This is a formula-based entitlement program; eligible state and territorial agencies receive annual allocations without competitive application.
