About this opportunity

Jefferson Parish, Louisiana is requesting Statements of Qualifications from licensed occupational medicine providers to serve parish employees and prospective employees, with submissions due October 30, 2026 at 3:30 p.m. through the Jefferson Parish e-Procurement system. Vendors must maintain an office on either the East Bank or West Bank of Jefferson Parish — a geographic eligibility requirement that filters the provider pool.

The scope covers pre-employment examinations, fitness-for-duty evaluations, physical examinations, specimen collection for drug and alcohol testing, medical treatment administration, and Medical Review Officer services for the parish substance use program. Minimum qualifications require a current Louisiana State Medical License in good standing and an office staffed with at least one occupational doctor, a minimum of two medical assistants, and one support staff member. All subcontractors must submit complete questionnaires with applicable licenses and required documentation.

Submissions must use the Jefferson Parish General Professional Services Questionnaire and the Human Resource Management Questionnaire, with cost information provided on Attachment B and staff names, titles, certifications, and licenses included. The parish may award contracts to one or more vendors. Evaluation allocates points across five categories: proposer qualifications (25 points), demonstrated ability to respond timely with qualified staff (25 points), demonstrated ability to provide services with experience serving populations of 2,000 or more employees and providing references (20 points), cost of services (20 points), and understanding of project scope (10 points). Certificates of insurance and affidavits are required upon contract approval. Disputes or protests are brought before the 24th Judicial Court per Jefferson Parish Code of Ordinance Section 2-932.
