About this opportunity

The State of Delaware Department of Health and Social Services, Division of Public Health is issuing a Request for Proposals to establish and operate Rural Community Health Hubs serving underserved communities in Sussex and Kent Counties. Vendors may propose to deploy and operate mobile health units (Component A), brick-and-mortar health hubs (Component B), or both components. Component A requires deployment of a minimum of four new mobile health units by Year 2 with at least 1,500 annual encounters by Year 3, while Component B requires establishment or expansion of health hubs achieving 1,500 annual patient encounters. Both components must achieve 70 percent or greater service delivery in high-need census tracts, maintain 65 percent closed-loop referral completion rates, and demonstrate integration with electronic medical records systems and the Delaware Health Information Network. Key evaluation criteria emphasize sustainability planning (weighted at 10 percent), relevant experience, quality assurance, organizational resources, partnerships, staffing qualifications, and cost effectiveness. The non-mandatory pre-bid meeting is scheduled for March 20, 2026, at 1:00 PM EDT via Microsoft Teams. Written questions are due February 27, 2026, at 4:30 PM EST, with responses posted by April 3, 2026. Proposals must be submitted April 17, 2026, at 1:00 PM EDT through the Euna Procurement Portal, with estimated award notification on May 29, 2026. The resulting contract will have an initial one-year term with four optional one-year extensions.

The RFP does not specify a total funding ceiling but requires vendors to submit detailed line-item budgets with narratives justifying all costs and demonstrating alignment with proposed activities and staffing. The State reserves the right to award to multiple vendors, reject any proposals, and cancel the solicitation. Vendors must carry commercial general liability insurance of $1,000,000 per occurrence and medical/professional liability or errors and omissions insurance of $1,000,000 per occurrence/$3,000,000 per aggregate. All vendors must complete standardized submission forms including non-collusion certifications, diverse vendor certifications at both state and federal levels, business capability statements, and identification of any proposed subcontractors. Vendors must maintain SAM registration and a Unique Entity Identifier (UEI), obtain criminal background checks for assigned personnel, and comply with comprehensive data protection and security standards including encryption of non-public data. The professional services agreement incorporates by reference a Business Associate Agreement for HIPAA and HITECH compliance, Department of Technology and Information terms and conditions governing data usage, and other required appendices, with the State retaining all data ownership and the right to terminate for convenience with twenty days' notice.
