About this opportunity

The Oregon Department of Human Services (ODHS), through its Office of Aging and People with Disabilities (APD), is seeking Nursing Facility Client Care Surveyors to conduct certification, recertification, and complaint surveys for nursing facilities statewide. This Request for Applications (RFA) requires qualified applicants to possess an active Oregon Registered Nurse license and be CMS-certified with SMQT qualifications, including completion of 18+ training courses exceeding 240 hours. Surveyors will be responsible for conducting 3-6 recertifications per month, handling complaint investigations, and submitting comprehensive monthly, quarterly, and annual reports using Aspen Software and the Long-Term Care Survey Process (LTCSP) tool. The work will be primarily remote with occasional in-office requirements in Salem and Tigard, Oregon. The RFA opens on December 23, 2024, with applications due by March 14, 2025 at 3:00 PM PST, and will be reviewed on a rolling basis every Friday at 3:00 PM PST after the first application is received. The initial contract term is up to three years, with potential extensions based on need and funding availability. Pricing submissions must include detailed line items for personnel costs, travel expenses, services, supplies, administrative costs, and indirect charges.

The state actively promotes equal opportunity and encourages participation from minority-owned, women-owned, emerging small businesses, and service-disabled veteran-owned businesses through the Oregon Certification Office for Business Inclusion and Diversity (COBID). Applicants must complete a Certified Disadvantaged Business Outreach Plan, detailing their certification status and potential subcontracting opportunities with Certified Firms. The RFA anticipates multiple contract awards but does not guarantee a specific number of contracts or minimum work volume. Contractors must meet comprehensive responsibility criteria, including submitting reference checks, disclosing past performance, and providing information about financial resources and legal history. Insurance requirements include Workers' Compensation and Commercial General Liability Insurance with minimum coverage of $1,000,000 per occurrence and $2,000,000 annual aggregate limit. While specific award values are not explicitly stated, the contract will reimburse only costs allowed under the Oregon Accounting Manual, with payment terms subject to standard state procurement regulations.
