About this file
This is an Attachment G - Certified Disadvantaged Business Outreach Plan form from the Oregon Department of Administrative Services (DAS) Procurement Services for RFP S#10000-00011335 regarding Nursing Facility Client Care Surveyors. The form is designed to collect information about an applicant's status as a certified disadvantaged business and their plans for subcontracting with other certified firms. The document requires applicants to indicate their certification status with the Oregon Certification Office for Business Inclusion and Diversity (COBID) across multiple categories including DBE, MBE, WBE, SDV, and ESB certifications.
The form requires applicants to provide information about their past relationships with Certified Firms and non-certified minority-owned, woman-owned, service-disabled veteran-owned or emerging small businesses over the previous two years. If subcontracting opportunities are anticipated, applicants must describe their plans to solicit Certified Firms, outline potential subcontracting opportunities with approximate dollar values, and indicate their willingness to report subcontractor identities and contract values to COBID. The document emphasizes equal opportunity participation for Certified Firms in state-funded contracts, though responses to this form are not considered in any scored evaluation.
