About this opportunity

Public Health Solutions is conducting a Request for Qualification (RFQ) for the WholeYouNYC Social Care Network (SCN) to pre-qualify independent contractors and organizations as Home and Asthma Remediation Direct Service Providers in Manhattan, Queens, and Brooklyn. The procurement seeks providers to deliver health-related social need (HRSN) services for Medicaid members, including home accessibility modifications, mold and pest remediation, ventilation improvements, equipment provision, indoor air quality services, and supportive products. Qualified applicants must be based in the United States, licensed in New York State, possess a Home Improvement Contractor License, and meet specific service-related credentials and certifications. The solicitation (#2025.08.SCN.07.01) has a response deadline of August 21, 2025, at 3 PM ET, with anticipated notification of acceptance in September 2025. The pre-qualified list will remain active until March 31, 2027, with potential annual updates, and is part of the New York State Department of Health's Health Equity Reform initiative aimed at screening, navigating, and delivering social care services to over 2.5 million eligible Medicaid members.

The RFQ encourages participation from New York State Certified minority- and women-owned businesses and service-disabled veteran-owned businesses. Providers will be reimbursed strictly on a fee schedule basis, with no advances or down payments, and must maintain appropriate insurance, comply with technical standards, and provide professional labor, equipment, and materials while meeting housing standards and codes. While specific incumbent vendors are not mentioned, the procurement aims to create a robust vendor network to support the SCN's goals of ensuring service quality, promoting transparency, and increasing efficiency in service delivery. The fee schedule for services is detailed in the attached document "Exhibit A - SCN HRSN Fee Schedule August 2025," which provides comprehensive pricing information for various health-related social services. The program emphasizes monitoring spending, preventing duplicative services, and ensuring efficient use of funding, with Social Care Navigators playing a key role in coordinating and managing these health-related social needs services for Medicaid members.

Public Health Solutions is seeking to pre-qualify independent contractors and organizations who meet the eligibility requirements to deliver home remediation and/or asthma remediation services for our WholeYouNYC Social Care Network (SCN). These services are critical to improving housing-related health outcomes for Medicaid members. Independent contractors and organizations with demonstrable experience in providing these services in New York City will be considered for inclusion on an SCN Pre-Qualified List (PQL). Those selected shall become pre-qualified partners that will be eligible for potential future engagements/assignments to provide these services which may include a requirement to enter into an agreement directly with a contracted HRSN Service Provider. This PQL will ensure a robust, responsive vendor network that can support the SCN's goal of delivering high-impact, housing-based interventions that improve the health and well-being of New York's Medicaid population. The Pre-Qualified List (PQL) will support the SCN's efforts to: • Ensure Service Quality and Compliance: Only providers with the appropriate credentials, training, and experience will be eligible to deliver remediation services. • Promote Transparency and Accountability: A vetted pool of service providers will help uphold consistent standards for safety, member health outcomes, and program integrity. • Increase Efficiency in Service Delivery: Pre-qualification will streamline the referral and contracting process, enabling faster response times to members with identified health-related social needs (HRSNs).
