The questionnaire requires disclosure of business experience, financial standing (current financial statements dated within six months of submission), equipment availability, project references, performance history with the City of Philadelphia, defaults or terminations, surety bond history, criminal convictions or investigations, pending litigation, and tax liabilities. Minority Business Enterprise (MBE), Women Business Enterprise (WBE), and Disadvantaged Small Business Enterprise (DSBE) designations are requested. The City retains the right to investigate all statements, contact references and financial institutions, and revoke pre-qualification status for capacity concerns. False information is subject to penalties under Pennsylvania law.
