About this opportunity

The Utah Department of Health and Human Services (DHHS) is soliciting proposals through a Request for Proposals (RFP) process for a five-year contract to deliver Projects for Assistance in Transition from Homelessness (PATH) services to seriously mentally ill individuals experiencing or at imminent risk of homelessness. The procurement seeks to award contracts with multiple qualified offerors to provide community-based outreach, mental health services, substance abuse treatment, case management, and limited housing support services. Deliverables include mandatory service delivery across nine core areas: outreach and identification services, screening and diagnostic treatment, habilitation and rehabilitation services, community mental health services, substance abuse treatment, staff training, case management, referrals for primary health services and job training, and housing assistance such as security deposits and one-time rental payments to prevent eviction. Key performance outcomes require contractors to enroll at least 65% of participants in community mental health services, contact individuals experiencing homelessness to contribute to a national target of 125,000 contacts, enroll at least 58% of contacted individuals in services, assist 10% of PATH enrollees with attaining housing, and ensure 80% of enrollees with clinically identified needs obtain mental health and substance abuse services. Eligible applicants must be either a 501(c)(3) non-profit organization or a local government entity (city, county, or municipality) in Utah with a mandatory minimum of three years' experience providing direct services to the homeless population. Proposals are due by March 30, 2026, submitted electronically through Utah Public Procurement Place (U3P), and must include a cover page with organizational information, a proposal abstract not exceeding 250 words, and fourteen scorable technical criteria sections totaling 200 points with a minimum threshold of 120 points (60%) required to advance in evaluation. The evaluation criteria assess service delivery capability across all nine service areas, community partnerships, participation in local homeless councils with veteran support, population reach assurance, and budget justification narrative, with outreach services and budget justification each weighted at 12.5 percent (25 points maximum).

Eligibility is restricted to non-profit organizations with active 501(c)(3) certification and local government entities; contracted support coordinators are ineligible to submit proposals unless their existing support coordinator contract will be terminated. All offerors must certify they are not debarred or suspended from federal contracts and maintain no outstanding Utah tax liens. Required submissions include proof of 501(c)(3) status or governmental entity designation, a completed IRS Form W-9, a Conflict of Interest Disclosure Statement (for non-profits) or Governmental Entity Conflict of Interest Certification (for government entities), and a Budget Utilization Form allocating costs across personnel and non-personnel categories. The maximum annual budget for each contract is $219,185, and contractors must establish a match requirement of at least $1 in non-federal contributions for every $3 of federal PATH funds received through cash or in-kind contributions. No more than 20% of federal PATH funds may be expended for eligible housing services, and administrative expenses are capped at 4% of federal funds. Contractors must complete Homeless Management Information System (HMIS) and PATH Data Exchange (PDX) training and submit quarterly data reports by October 31, January 31, April 30, and July 30, with monthly invoices submitted through an electronic billing system and all state fiscal year billings due by July 14 of the following year.
