The scope is manufacturing two items: Item 1 (Medicaid—NBS3 devices) with a fiscal year 2027 estimated quantity of 422,400 units, and Item 2 (Paid—NBS4 forms) with 354,000 units. Forms must include sequential numbering from 27-0000001 through 27-0422400 with guaranteed accuracy, device serial numbers, barcodes, expiration dates, NBS QR codes, lot numbers, and revision dates. Detailed specifications for check digit background colors, filter paper lot numbers, and kit type designations are supplied; final text and screen color determinations will be established collaboratively with the selected vendor. Bidders must comply with current good manufacturing practice (CGMP) requirements and FDA regulations for medical devices and maintain comprehensive quality assurance protocols with detailed QA data packages for each shipment.
Bidders must demonstrate a minimum of two years of relevant experience and provide three verifiable references for similar services within the last two years. Pricing must be submitted as firm, fixed prices on the provided pricing sheet (Exhibit C), including all labor, equipment, materials, supplies, and related expenses. No minimum volume or compensation is guaranteed.
The initial contract term runs from September 1, 2026 to August 31, 2027, with four optional one-year renewals through August 31, 2031. Delivery is scheduled in two shipments: 211,200 Medicaid devices and 177,000 Paid devices by February 1, 2027, and the same quantities by July 1, 2027. Historical reference value for the period September 1, 2023 to March 31, 2026 was $870,173. The Texas Health and Human Services Commission (HHSC) is procuring on behalf of DSHS.
