About this opportunity
The Illinois Department of Public Health is seeking a sole-source contractor to operate a gift card rewards management system for the Pregnancy Risk Assessment Monitoring System (PRAMS) survey, with proposals due June 15, 2026. The contract awards a $25 gift card to each survey respondent for restricted purchase of baby diapers and baby wipes from participating retailers throughout Illinois.
This is a small purchase under sole-source conditions, which narrows competitive participation. The awardee must establish and manage the gift card platform, including vendor enrollment, redemption processing, and compliance with product-category restrictions.
Vendors must comply with 34 mandatory State of Illinois certifications covering legal status, ethics, drug-free workplace, equal employment opportunity, cybersecurity clearance, domestic product preferences, and other eligibility standards. False certifications or non-compliance trigger contract voidance, suspension, debarment, or civil and criminal penalties under the Illinois False Claims Act. Vendors with contracts exceeding one year must recertify annually by January 1.
Contracts valued at $1,000 or more are subject to a $15 Minority Contractor Initiative fee deductible from the first payment. General commercial liability insurance of $1 million per occurrence and $2 million annual aggregate, auto liability of $1 million per occurrence, and workers' compensation coverage as required by law are mandatory. Prevailing wage compliance applies to applicable service categories per Illinois Department of Labor rates.
The State retains termination rights for cause immediately or for convenience with thirty days' notice, and reserves the right to suspend or terminate if the Illinois General Assembly fails to appropriate sufficient funds. All work products are deemed works for hire with exclusive ownership vesting in the State. Record retention is three years, or five years if federal funds are used, and records remain subject to audit by State agencies and the Auditor General.
