About this opportunity
The Illinois Department of Child and Family Services is soliciting a small purchase on a sole-source basis to obtain copies of medical records for children entering the child welfare system under PO27-053, with a due date of May 28, 2026 (same-day submission).
Sole-source justification applies; the procurement does not open to competitive bidding. Vendors must complete the Standard Illinois Certifications form (Version 26.2), which establishes 34 distinct compliance requirements covering legal eligibility, financial responsibility, ethical conduct, and workplace standards. Material certifications include absence of criminal convictions for bribery or felonies within five years, no environmental violations, non-delinquency on state debts, tax compliance including Illinois Use Tax remittance, drug-free workplace status, equal employment opportunity policies, prevailing wage compliance where applicable, non-participation in international boycotts, and compliance with restrictions on forced labor and child labor in foreign-made products. False certifications expose vendors to contract voidance, suspension, debarment, civil fines, and criminal penalties under the Illinois False Claims Act. Subcontractors must be included in the contract and meet identical certification requirements.
Contracts exceeding one year in duration require annual recertification by January 1, and vendors must report any changes affecting eligibility status throughout the contract term. The state retains all work product as work-for-hire. Vendors must maintain general commercial liability insurance of $1 million per occurrence with the state named as additional insured, obtain prior written approval for any subcontractors, and comply with prevailing wage standards where applicable. The state reserves the right to terminate for cause with written notice, for convenience with 30 days' notice, or if appropriations become unavailable. All disputes proceed exclusively through the Illinois Court of Claims; binding arbitration is not available.
