About this opportunity

The Commonwealth of Virginia Department of Behavioral Health and Developmental Services, procuring for Piedmont Geriatric Hospital, is requesting proposals for on-site optometry clinic services with sealed proposals due September 16, 2026 at 3:00 PM (RFP #127020). The Commonwealth encourages participation from DSBSD-certified small businesses, women-owned businesses, and minority-owned businesses, though no set-aside or participation goal is stated as mandatory.

The scope requires a Virginia-licensed, board-certified optometrist credentialed as a Medicare/Medicaid participating provider to deliver general optometry services—vision exams, refraction, prescriptions, glaucoma screening, and diabetic eye exams—on-site at the Burkeville facility for approximately 12 patients per clinic session, with option to serve additional patients by mutual agreement. Services target approximately 123 residents, all age 65 and older with mental illness. The contractor must maintain professional liability insurance at $1 million per occurrence and $3 million aggregate, execute a Business Associate Agreement for HIPAA compliance, and obtain criminal background clearance. Performance standards include timely arrival with penalties for late arrivals and no-shows.

Pricing is an all-inclusive flat rate per optometry clinic for up to 12 patients, billed monthly based on services rendered, with payment due within 30 days of invoice receipt. Historical annual spending is below $10,000. The initial contract period runs October 1, 2026 through September 30, 2027, renewable for four successive one-year periods. The incumbent is A to Z Eye Care.

Evaluation allocates 100 points across five criteria: experience with behavioral health residents (20 points), experience providing required services (20 points), clinic availability and scheduling (20 points), comprehension of medically necessary care (20 points), and small business subcontracting plan (20 points). The Commonwealth may terminate with 60 days written notice.
