The Affirmation and Disclosure Form prohibits offshore services, offshore data location, and Russian sourcing. All service and data locations must be disclosed and remain within the United States unless the state grants a waiver. Contractors must affirm compliance with Ohio IT policies and standards, including cloud service requirements through the State IT Cloud Smart Strategy and integration with enterprise services such as the InnovateOhio Platform and Microsoft 365 services.
