About this opportunity
The Colorado Department of Human Services issued a Documented Quote (DQ) for the FY16 Optometry Upgrade project to replace existing optometry equipment at the Colorado Mental Health Institute at Fort Logan in Pueblo County. The procurement encompasses nine categories of specialized optometry equipment, including an optometry chair, slit lamp with tonometer and mount, phoropter, instrument stand with console, binocular indirect ophthalmoscope, hand-held instruments (ophthalmoscope, retinoscope, transilluminator, and rechargeable handles), computerized vision system, lens-meter, and protective dust covers. The successful vendor must provide all delivery, installation, assembly, testing, calibration, and warranty repairs on-site, with all equipment and parts being new and containing manufacturer warranties. Vendors were required to submit manufacturer cut sheets and warranty information with their bids, and failure to do so could result in disqualification. Quotes were due by November 17, 2015 at 10:00 AM via email to darla.geer@state.co.us or fax to 719-546-4896, with the award period running from execution through June 30, 2016 on a non-renewable basis. All pricing must be quoted FOB Destination with firm pricing for the award term and payment terms no less than 45 calendar days, with price and cost serving as the primary evaluation criteria.
Service Disabled Veteran Owned Small Businesses (SDVOSBs) registered with the U.S. Department of Veterans Affairs were eligible to receive a 5% preference on their bids. The State reserved the right to increase or decrease quantities based on available funding and intended to issue a single vendor award based on which acceptable response proved most advantageous to the State. The awarded vendor was required to extend the negotiated pricing to other Human Services divisions located in Pueblo and Grand Junction, effectively creating a broader pricing agreement across multiple locations. No specific budget range or award value was disclosed in the solicitation documents, and no incumbent vendors or existing infrastructure integration requirements were noted.
