About this opportunity

The Michigan Department of Health and Human Services is requesting proposals for in-home family preservation services in Alpena and Montmorency Counties under RFP 260000002008, with proposals due June 10, 2026 at 3:00 p.m. Eastern and bidder questions due May 27, 2026 at 3:00 p.m. Eastern.

The scope comprises in-home family preservation services including face-to-face client contacts, travel to and from client locations, and travel for unsuccessful contact attempts. Workers must maintain flexible schedules accommodating non-traditional hours outside Monday–Friday 8:00 a.m.–5:00 p.m., including evenings and weekends. Contractors must submit weekly face-to-face contact lists to referring MDHHS workers via secure email and may be required to provide testimony in court proceedings. Upon completion of the initial six-month service period, additional services outside the contract scope may be approved if deemed necessary and eligible by the referring MDHHS worker. Contractors must hold minimum insurance limits of $1,000,000 Commercial General Liability and $1,000,000 Automobile Liability.

The contract amount is $300,000 over three years (October 1, 2026 through September 30, 2029), allocating $100,000 per fiscal year, with potential renewal for up to two additional one-year periods at the State's sole discretion. Bidders propose an hourly unit rate on Schedule B Pricing Matrix covering all direct and indirect costs including mandated insurance. The pricing structure includes $15,000 annually for transportation actual costs and $3,000 annually for specific assistance services, reimbursed separately at the state premium mileage rate or the contractor's usual employee reimbursement rate, whichever is less. Invoicing occurs monthly using the MDHHS-5974 form as services are provided.

Technical evaluation totals 125 points: Experience and Qualifications (15 points), Delivery Capability (40 points), Service Capability and Understanding (40 points), and Instructions for Bidders and Vendor Questions Worksheet (30 points). Proposals scoring 100 or more technical points advance to pricing evaluation. The State will determine award based on best value combining technical merit and pricing and is not obligated to accept the lowest price. All proposals must be submitted electronically through the Michigan SIGMA VSS system in modifiable formats.
