About this opportunity

Walworth County is requesting proposals for licensed Clinical Dietitian services at Lakeland Health Care Center (LHCC), a 90-bed long-term care facility in Elkhorn, with proposals due August 17, 2026 at 10:00 a.m. via the Bonfire Portal and questions due August 4, 2026 at 3:00 p.m. The central eligibility requirement is Wisconsin certification or licensure as a Dietitian per Wisconsin Statutes 448.78 or equivalent state certification, with three years of verifiable long-term care dietitian experience, Wisconsin business licensing, non-debarred status, and specified insurance coverage (Commercial General Liability $1 million per occurrence / $2 million aggregate; Commercial Automobile Liability $1 million combined; Professional Liability / Medical Malpractice $1 million per occurrence / $3 million aggregate).

The scope calls for approximately 32 hours of monthly clinical dietitian services, including new resident nutritional assessments, medical record and laboratory review, monthly weight and wound-care meetings, monthly care planning, quarterly and annual reassessments, individualized meal plan development, malnutrition risk identification, interdisciplinary care team collaboration, regulatory compliance documentation, survey preparedness assistance, resident and staff education, and menu approval oversight.

The contract term is two years commencing November 1, 2026 through October 31, 2028, with three optional one-year renewal periods under the same terms by mutual agreement. Pricing shall be submitted as an all-inclusive hourly rate covering labor, travel, mileage, administrative costs, and all expenses with no additional fees or surcharges permitted. Labor costs are firm for the first contract year; thereafter, annual adjustments are capped at 3% with 30 days' written notice, based on the Consumer Price Index for All Urban Consumers (Medical Care Services Index, Midwest Region). The county assumes no obligation to purchase a minimum amount of services. Payment is on a cost-reimbursement basis within 45 days of invoice receipt, with the county preferring procurement card payment. As an independent contractor, the selected firm is not eligible for county employee benefits and must comply with HIPAA and HITECH Business Associate requirements.

Evaluation criteria award 65 points for technical proposal qualifications (firm experience, staff credentials and licensing, scope comprehension, value-added services) and 25 points for cost. The county anticipates award on September 17, 2026. Walworth County reserves the right to reject any or all proposals and permits piggyback purchasing by other governmental units at identical terms.
