About this opportunity

Michigan's Department of Technology, Management & Budget is soliciting bids for printing 12,500 DI-93 Officer's Refusal to Submit to Chemical Test forms, organized into 500 packages of 25 forms each, with bids due September 4, 2026. Only prequalified vendors registered with the State of Michigan are eligible.

The scope is production of four-ply NCR (carbonless) forms at 8.5" × 11" upright, with four designated plies (driver's copy for hearing, driver's copy with notice of suspension, and two officer's copies on pink paper). Front pages print in 100% black ink; back pages at 50% screen black ink. Forms must be bound as NCR pads at the top, shrink-wrapped in packages of 25, and packed in cartons on four-way skids with a maximum weight of 40 pounds per carton. Bidders must submit detailed quote letters with their bids and provide one PDF proof for approval before production begins; failure to submit a quote letter results in disqualification.

Delivery is F.O.B. Destination, prepaid, to the state dock at 7461 Crowner Drive, Lansing, Michigan 48913 (Monday–Friday, 8:00 a.m.–3:30 p.m., excluding standard holidays). All shipping costs and risks are borne by the vendor until final state acceptance. The state maintains an overs/unders policy with no underruns accepted and overruns at the vendor's expense. All materials and electronic copies become property of the state and must be returned upon request in source software format (including all revisions, fonts, and images) and PDF format.

The state awards based on best price and value rather than lowest bid alone, considering past quality performance and ability to meet specifications. Invoices must be emailed to DTMB-PrintingServices@michigan.gov and entered into the VSS system. Payment of undisputed amounts is due within 45 calendar days of invoice receipt or deliverable acceptance, processed via Electronic Funds Transfer. Vendors must replace or correct defective deliverables within ten calendar days or face rejection, discounted acceptance, or state-directed correction at vendor expense plus administrative fees.
