About this opportunity

The State of Michigan's Department of Technology, Management & Budget is soliciting printing services for 250,000 copies of the three-part carbonless CHJ 549 Health Care Request Form, with quote letters due November 6, 2026. Only prequalified vendors are eligible, and the State's Print & Mail Management terms require registration with the State, EFT payment acceptance, and PMM pre-qualification before bid submission. The terms state that pre-qualification typically takes approximately one week, which leaves little time before the due date.

The scope covers 8.5-by-11-inch portrait forms with identical printing on Parts 1 and 2 and different printing on Part 3. Part 2 requires microperforations across the 8.5-inch dimension, 5/16 inches from the top, and the NCR pad is assembled at the top. Packaging requires plain unmarked cartons labeled only with form number and quantity, with copies shrink-wrapped in sets of 100 on chipboard backing, 10 packages of 1,000 copies per carton, and four-way skids. The bid request specifies delivery to Jackson, Michigan 49201. The PMM terms require freight-inclusive bids to the State dock at 7461 Crowner Drive, Lansing, Michigan 48913, on an F.O.B. destination, prepaid basis. Pallets are hardwood, with a 4,000 gross-pound maximum and dimensions of 44 to 48 inches long, 36 to 48 inches wide, and no more than 5'6" high. Dock delivery is limited to 8:00 a.m. to 3:30 p.m., Monday through Friday, excluding standard holidays. A PDF proof requires State approval before production, and electronic files and PDF samples are provided to bidders.

Detailed quote letters are mandatory, and omitting them results in automatic disqualification. Awards are made on best price and value to the State, so the lowest bidder may not receive the award if quality or specification concerns exist. Late bids are not accepted, and the vendor bears the cost of any overrun under the no-underrun policy.

Standard terms include F.O.B. destination delivery, a 30-day final inspection period, and payment within 45 calendar days of invoice or final acceptance, whichever is later. Vendors must correct defective deliverables within 10 calendar days or face State-performed corrections billed at cost plus a 10% administrative fee. Records must be retained for four years after termination. Michigan Prevailing Wage Act provisions apply where applicable. Invoices are emailed to DTMB-PrintingServices@michigan.gov and entered into the VSS system, with inquiries directed to Andrew Hale at 517-636-0092 or halea3@michigan.gov.
