About this opportunity

The Commonwealth of Kentucky Finance and Administration Cabinet, on behalf of the Cabinet for Health & Family Services and specifically the Office of Children with Special Health Care Needs (OCSHCN), is soliciting bids through Request for Bid (RFB) 758 2400000320 for a comprehensive Master Agreement to supply audiology supplies to state agencies serving a pediatric population aged 0-21 years across eleven clinic locations throughout Kentucky. The procurement encompasses approximately 700+ line items including hearing aid components (tubing, earmolds, desiccants, batteries), diagnostic equipment accessories (probe tips, electrodes, eartips), cleaning and maintenance supplies, impression materials, storage solutions, and educational materials. Bidders must submit pricing on all line items without exception, or their bids will be deemed non-responsive; certain items are brand-specific with no substitutions permitted (including Welch Allyn Diagnostic products and 3M Double Stick Contour Tape), while other items allow for brand or agency-approved equal substitutes if product information is documented. The initial contract period is one year from the date of award, with four optional one-year renewal periods available by mutual written agreement. Award will be made on a best value basis using measurable criteria weighted as follows: price (90 points maximum) and delivery in working days (10 points maximum), with the vendor offering the lowest total price receiving maximum price points and others scored proportionally. Written vendor questions must be submitted by January 10, 2024, at 10:00 a.m. ET, and bids are due electronically through the Kentucky eProcurement system by January 17, 2024, at 1:30 p.m. ET; paper submissions are not acceptable. All pricing must remain valid for a minimum of 60 calendar days after the bid due date, with prices quoted F.O.B. Destination Freight Prepaid and Allowed, exclusive of federal excise taxes and Kentucky sales and use taxes.

Kentucky resident bidders receive reciprocal preference against nonresident bidders, and qualified bidders including Kentucky Correctional Industries, Kentucky Industries for the Blind, and qualified nonprofit agencies for individuals with severe disabilities meeting specified employment thresholds and safety standards receive additional preference points equal to 20 or 15 percent of maximum points awarded. All bidders must complete a notarized Annual Affidavit for Bidders, Offerors and Contractors (Attachment B) certifying compliance with Kentucky campaign finance laws, and foreign entities must provide a Certificate of Authority before contract award. Bidders claiming resident bidder status must affirm they are authorized to transact business in Kentucky and have maintained Kentucky corporate income tax filings, unemployment insurance fund payments, and workers' compensation coverage for at least one year prior to contract advertisement. Price increases are not permitted during the first twelve months of the contract; any subsequent increases require written justification based on industry-wide price changes and must be requested at least 30 days in advance, while price decreases must be reflected immediately upon availability. A one percent daily deduction from invoices applies for each day delivery exceeds the contracted due date. No incumbent vendor or budget range has been specified in the solicitation documents; however, the estimated annual purchase quantities for individual line items range from 1 to 110 units, with individual item pricing ranging from $0.25 to $485.00, suggesting a contract value that will be contingent upon vendor pricing and actual usage patterns across the eleven designated Kentucky clinic locations.
