About this opportunity
The Commonwealth of Kentucky Finance and Administration Cabinet, on behalf of the Cabinet for Health & Family Services, is soliciting bids for a Master Agreement to supply comprehensive audiology supplies to the Office of Children with Special Health Care Needs serving pediatric patients aged 0-21 years across eleven clinic locations throughout the state. This Request for Bid (RFB 758 2400000228) seeks a single vendor to supply approximately 850 line items of audiology equipment and supplies, including earmolds and ear dams, hearing aid batteries and dehumidifiers, cleaning and maintenance supplies, diagnostic equipment accessories, tubing and hardware, eartips, electrodes, bone conduction equipment, and impression materials. The contract will be awarded on a group basis, meaning all items will be awarded to a single supplier, with vendors required to bid all items or face rejection as non-responsive. The initial contract term is one year from the date of award with four optional one-year renewal periods subject to written approval and potential renegotiation. Vendors must submit bids electronically through the Commonwealth eProcurement system and be registered in the Vendor Self Service System prior to submission. The solicitation was released on November 7, 2023, with written vendor questions due by November 15, 2023, at 10:00 a.m. ET, and proposals due by November 28, 2023, at 1:30 p.m. ET. Award will be made on a best value basis using measurable criteria of Price (90 points) and Delivery (10 points), with pricing quoted on an F.O.B. Destination Freight Prepaid and Allowed basis including all shipping costs.
Kentucky resident bidders are eligible for reciprocal preference consideration, and qualified bidders including Kentucky Correctional Industries and other qualified nonprofit agencies may receive scoring adjustments of 20 percent and 15 percent, respectively. Certain brands are designated as brand-specific with no substitutions allowed, including Welch Allyn Diagnostic equipment and 3M Double Stick Contour Tape, while other items such as Loctite 401 Adhesive, Westone Pre Bent products, Adco-Ment, Gemoro, Rayovac, Wagner, and Westone materials are acceptable as brand or agency-approved equals. Vendors must comply with all applicable Kentucky statutes and provide required affidavits and tax registration documentation prior to award. Estimated annual purchase quantities for individual line items range from 1 to 110 units, with delivery required to ten specified clinic locations throughout Kentucky including Barbourville, Bowling Green, Elizabethtown, Hazard, Lexington, Louisville, Morehead, Owensboro, Paducah, Prestonsburg, and Somerset. Price increases are prohibited during the first twelve months of the contract and thereafter require written justification based on industry-wide price changes. No incumbent vendor information or specific budget range has been disclosed in this solicitation.
